Starting in 2024/2025, the UK Medical Licensing Assessment (UKMLA) became the required standard for all doctors joining the GMC register. At Global Med Tutor, we provide a structured, evidence-based curriculum specifically designed to help you navigate both the Applied Knowledge Test (AKT) and the Clinical and Professional Skills Assessment (CPSA).
UKMLA, AKT, CPSA and PLAB: how they actually relate
The Medical Licensing Assessment has two components: the Applied Knowledge Test and the Clinical and Professional Skills Assessment. Graduates of UK medical schools must pass the MLA before joining the medical register. International medical graduates do not sit the MLA itself — they sit PLAB, which the GMC has made compliant with the same requirements: PLAB 1 meets the AKT requirements and PLAB 2 meets the CPSA requirements.
- PLAB has not been renamed. It retains its name and remains the route for international graduates; the GMC has said it will be modernised at a later date.
- Same topics, same standards. Both groups are assessed against the same MLA content map and the same delivery requirements.
- Requirements govern the test, not the candidate. The GMC is explicit that these requirements describe how assessments are built and delivered, not how individual performance is judged.
Why the UKMLA Requires a Unique Approach
The UKMLA is not just a test of medical knowledge; it is an assessment of your ability to practice safely within the UK healthcare system. Our tutoring aligns with the MLA Content Map, focusing on:
- Professionalism & Ethics: Navigating GMC Good Medical Practice guidelines.
- Clinical Reasoning: Moving beyond rote memorisation to high-level diagnostic logic.
- Patient-Centred Care: Emphasising the communication skills required for the UK clinical environment.
Applied Knowledge Test (AKT) Mastery
The AKT is a computer-based test of applied clinical knowledge, built and delivered against the GMC’s published AKT requirements and the MLA content map. We help you conquer the AKT through:
- Core Clinical Knowledge: Intensive review of the most common presentations in the MLA Content Map, including Acute Medicine, Surgery, GP, and Mental Health.
- Data Interpretation: Specialised training on interpreting lab results, ECGs, and imaging within the context of UK clinical guidelines (NICE/SIGN).
- SJT & Pharmacy Integration: Mastery of Situational Judgement and prescribing safety, which are critical components of UK practice.
Clinical & Professional Skills Assessment (CPSA)
For the CPSA, we focus on the practical application of your skills.
- Station Strategy: We break down the “anatomy” of a CPSA station—from history taking to clinical examination and explanation.
- Communication Skills: Personalised coaching on “The Calgary-Cambridge Model” to ensure you build rapport and involve patients in shared decision-making.
- Mock Assessments: Realistic 1-on-1 simulations with immediate feedback on your clinical technique and professional demeanour.
The Global Med Tutor MCQ & Strategy Advantage
Success on the UKMLA depends on your ability to handle “best of five” questions effectively. We don’t just teach you the facts; we teach you the GMC logic.
Our Strategy Training Includes:
- Thinking Like an Examiner: We work with you to create and deconstruct MCQs, helping you identify the “key features” of a clinical case that the GMC expects you to recognise.
- Time Management: Strategies for maintaining accuracy under the pressure of a 4-hour exam window.
- Evidence-Based Revision: Integrating spaced repetition and active recall to ensure long-term retention of the MLA Content Map.
UKMLA preparation: common questions
Is the UKMLA the same as PLAB?
No, but they are held to the same standard. UK graduates sit the MLA; international graduates sit PLAB, which the GMC has made compliant with MLA requirements. PLAB 1 meets the Applied Knowledge Test requirements and PLAB 2 meets the Clinical and Professional Skills Assessment requirements. The PLAB name has been retained.
Has PLAB been replaced by the AKT and CPSA?
No. This is a common misreading. PLAB 1 and PLAB 2 were not renamed; they were brought into alignment with the AKT and CPSA requirements during 2024. Candidates still book and sit PLAB under its existing name.
What does the MLA content map cover?
The GMC publishes a content map setting out the areas both the AKT and the CPSA are built against, spanning presentations and conditions across acute and chronic care, alongside professional capabilities. We map preparation directly to it rather than to a generic syllabus.
How much does the AKT test UK-specific practice?
Substantially. The assessment is concerned with safe practice in the UK healthcare context, which means national guidance, prescribing safety and professional and ethical judgement carry real weight — not only clinical recall. Candidates trained outside the UK usually need targeted work here, and we build that in.
What is assessed in the CPSA?
Clinical and professional skills in a station-based format — history taking, examination, communication, and explanation to patients. Performance depends as much on structure and shared decision-making as on clinical knowledge, which is why we coach station technique separately from content.
How long should UKMLA preparation take?
It depends on baseline performance and whether the candidate trained in the UK. Rather than work to a fixed calendar, we start with a diagnostic assessment against the MLA content map so your time goes to the areas that are genuinely weak.
What happens in the free introductory session?
We use it to discuss your exam strategy and timeline, work out where your clinical reasoning is breaking down, and let you see how we actually teach before you commit to anything. It is a conversation about approach, not a sales call.
Is support available after a failed attempt?
Yes. After an unsuccessful attempt we start by identifying the specific reasoning or technique failures behind it, rather than restarting the syllabus, and we run a dedicated exam resit and recovery track for candidates in this position.
